NDIS Physiotherapy
Goals-focused, exercise-based physiotherapy home visits for NDIS participants across Greater Melbourne, with hydrotherapy where it suits the plan.

Personalised, goals-focused exercise-based physiotherapy for NDIS participants experiencing reduced mobility, balance, strength or coordination. Our physiotherapy home visits bring treatment to you across Greater Melbourne, with one-to-one hydrotherapy available when it suits your goals.
A plan only works if the support behind it does
An NDIS plan sets out goals. It does not achieve them on its own.
When physiotherapy is hard to get to, sessions get missed. When a provider rotates clinicians, trust resets every visit. When a program is built for a clinic instead of a life, exercises get left in a drawer. Progress stalls, and the goals in the plan sit still with it.
For participants working on mobility, strength, balance or coordination, consistency is everything. Function builds slowly through the right exercises, done regularly, progressed at the right pace. Stop-start support does not just slow things down. It can mean losing hard-won capacity, and with it, confidence and independence.
The right physiotherapy relationship looks different. One team that knows you. A plan written around your goals and your actual daily environment. Clear communication with your support coordinator, so everyone can see the progress happening. That is what Upward is built to provide.
Support built around your goals
NDIS presentations vary enormously, and we treat them that way. Two participants with the same diagnosis can need completely different programs. Your plan starts from your goals, not a template. We are best suited to participants who want active, exercise-based physiotherapy. Our focus is adults. If you are enquiring for a younger participant, get in touch and we will tell you honestly whether we are the right fit. [CONFIRM WITH SAM: minimum age to list]
A real story: physiotherapy that was easier to access
A 60-year-old wheelchair user was experiencing significant shoulder pain from the physical demands of using his chair. Travel to a physiotherapy clinic was frustrating, with accessibility difficulties and narrow ramps. As he put it: “You’d expect a physio clinic to be accessible… apparently not.”
We began home visits and assessed his strength, identifying weakness in the shoulder’s rotator cuff. A six-week shoulder strengthening program at home helped improve his pain and made it easier for him to get out into the community.
Working with your support team
With your consent, we keep the people around you in the loop: support coordinators, plan managers, carers, family and other providers. Depending on what the participant and their team need, that can include initial assessment findings, functional goals, treatment recommendations, progress updates and reporting to support plan reviews. We agree what is useful at the start, so referrers get what they need without chasing.

How physiotherapy may support NDIS goals
Our NDIS approach
We start with your goals, assess function in your environment, build a clear plan and adapt it as your capacity grows.
A short check-in.
We spend the first five to ten minutes discussing how you have been, what has changed and how you have managed with your exercises.
A review of your movement and exercises.
Your physiotherapist checks how you are moving and how your current exercises are going, then makes adjustments where needed.
Targeted exercise.
The main part of the session focuses on exercises chosen for your goals and assessment. If one hip is weaker, we may work on its strength and control.
A clear wrap-up.
In the final five to ten minutes, we summarise your progress, confirm your next visit and agree on what to focus on between sessions, with your physiotherapist’s guidance.
Clinic travel is one of the biggest barriers to consistent NDIS physiotherapy. A physio home visit removes that barrier, and it changes what therapy can do. For many participants, home physio is the most practical way to keep treatment consistent.
When we provide physiotherapy home visits, we work on the movements your day actually involves. Your front step. Your bathroom. Your kitchen, your street, your local gym. Treatment becomes practice for real life, not rehearsal in a room you visit once a week.
We make exercises and goals relevant to the places, routines and activities that matter to you. For participants who benefit from variety, sessions can also happen at a gym or a hydrotherapy pool when that suits the plan.
