Exercise rehabilitation built around your real life
Structured, progressive exercise programs built around your home, your ability and your goals. The foundation of most of the work we do: rebuilding strength, mobility and confidence where daily life happens.

Upward Physiotherapy provides personalised exercise and rehabilitation through physiotherapy home visits across Greater Melbourne for older adults, NDIS participants, Home Care Package and Support at Home clients.
Exercise rehabilitation starts with the real problem
Exercise rehabilitation is not about handing you a generic list of exercises. It starts by identifying what is actually limiting your movement and daily function.
That might be reduced strength, poor balance, limited flexibility, low endurance, difficulty walking or another movement problem. We then prescribe exercises designed to target that specific impairment and help restore the function it is affecting.
The goal is practical progress: making it easier to stand from a chair, move safely around your home, walk further, manage the front step or get back to activities that matter to you.
What exercise and rehabilitation physiotherapy includes
Every program starts with a home visit physiotherapy assessment. We look at your goals, movement, strength, balance, mobility, walking ability and endurance, as well as the way you move through your home.
Those findings guide the plan. Depending on what is limiting you, your program may include:
Strengthening to improve force, stability and everyday movement
Balance exercises to help you move more safely and confidently
Flexibility and mobility work to make movement easier
Cardiovascular or endurance training, including walking or cycling
Functional practice based on the tasks you need to do at home and in the community
We explain what we find, what we recommend and how the program will progress. You know what we are working on and why it matters.
The benefits are measured in daily life
The benefit is not simply having stronger muscles. It is being more capable in the situations that matter to you.
Stronger legs can make standing from a chair easier
Better balance can help you feel safer in the shower or at the front step
Improved endurance can help you walk further without needing to stop as often
Better mobility can make it easier to get outside, garden or move around your home
Greater stability and capability can support confidence with everyday activity
Removing physical barriers can help you work toward your personal, NDIS or aged care goals
Your goals shape the program, and your progress is reviewed as you improve.
A real story: strength that changed daily life
An 83-year-old woman was referred for in-home physiotherapy after blocked arteries in her legs restricted blood flow to her calf muscles. She had never done strength training and had developed significant weakness and muscle loss. Clinical testing placed her in a high-risk category for falls, and she had little confidence with walking or exercising at home.
We began with a simple home exercise program and gradually progressed her strengthening under a physiotherapist’s supervision. Her strength improved dramatically. Follow-up clinical testing no longer placed her in the falls-risk category, her walking became much faster, and she now feels confident moving around her home and garden. She enjoys keeping up her exercise program and says she can see her calf muscles for the first time in years.
How Upward helps
What a physiotherapy home visit looks like
Sessions happen at home, but they are not limited to your living room. Where it suits your goals, we may work in your garden, on your street, at a local gym or in a hydrotherapy pool.
A short check-in.
We spend the first five to ten minutes discussing how you have been, what has changed and how you have managed with your exercises.
A review of your movement and exercises.
Your physiotherapist checks how you are moving and how your current exercises are going, then makes adjustments where needed.
Targeted exercise.
The main part of the session focuses on exercises chosen for your goals and assessment. If one hip is weaker, we may work on its strength and control.
A clear wrap-up.
In the final five to ten minutes, we summarise your progress, confirm your next visit and agree on what to focus on between sessions, with your physiotherapist’s guidance.
Home visit physiotherapy lets you learn and practise exercises in the environment where you will actually do them.
The chair you use every day can become your strength equipment. The hallway you walk through each evening can become your training space. This makes the program easier to understand and easier to repeat between visits, so exercise can become part of your routine rather than something that only happens during an appointment.
You can practise the exercises several times a week in the same space where they were taught. That removes the guesswork of going home from a clinic and trying to recreate the movement yourself.
